Job summary

Wayspring seeks a dedicated Member Engagement Specialist to support enrollment and ongoing engagement for health plan members in the Cedar Rapids area. You will travel to facilities and communities to educate members about services and secure engagement throughout their recovery journey.

The role emphasizes strong communication, collaboration with discharge teams, and ongoing follow-up to ensure members access clinical services and maintain engagement for up to 90 days post-discharge.

Qualifications

• Minimum of three (3) years of experience in an outreach, social work, or street outreach role.
• Strong communication and presentation skills; ability to de-escalate crises.
• Experience with CRM platforms or Electronic Health Record systems (EHR/EMR) preferred.

Responsibilities

• Facilitates in person enrollments at Wayspring facilities or through provider outreach.
• Educates members on Wayspring services to support enrollment.
• Secures consent and paperwork for services from members to participate.
• Collaborates with discharge planning teams to ensure safe, stable post-discharge plans.
• Schedules first clinical appointments and follows up 30–90 days post-discharge.
• Develops strategies to increase member contact, including in-person engagements.
