The role supports medication services, health monitoring, patient education, care coordination, and continuity of care under RN supervision and collaboration with psychiatrists and the treatment team. Qualifications

• Collects health information and vital signs and reports changes.
• Administers prescribed medications and documents response and side effects.
• Provides basic nursing care within LVN scope and plan of care.
• Educates patients on medications, wellness, and relapse-prevention.
• Coordinates care with RN, psychiatrists, and treatment team.

Responsibilities

• Travel to patient homes and community locations to provide services.
• Collaborate in multidisciplinary treatment team meetings and care planning.
• Document nursing services and ensure regulatory compliance.
• Follow LVN scope of practice and agency policy.
• Monitor and report safety issues and changes in patient status.
