UnitedHealth Group's UnitedHealthcare unit seeks a Field Based HSS Coordinator to relay information about member needs and advocate for the best possible care in an Integrated Care Model. This is a field-based role with a home-based office in the Dallas area.

You will complete home assessments, travel up to 75% (within 100 miles of Dallas), coordinate services, and manage documentation. Hybrid scheduling available if within 100 miles.

Qualifications

  • Must have one of the following: High School Diploma/GED (or higher) AND 5+ years of experience working with ABD/SSI population within a clinical health care role
  • Associate's degree with 3+ years of clinical experience
  • Unrestricted LPN/LVN license in the state of TX with 3+ years of clinical experience
  • CHW certification with 3+ years of clinical experience
  • Intermediate level of proficiency with Microsoft Office (Word, Excel, Outlook)
  • Access to reliable transportation and ability to travel up to 75% of the time in this assigned area to visit Medicaid members in homes or other settings
  • Must be willing to use approved assessment technologies, including AI-assisted tools, to perform assessments

In thisField based HSS Coordinator role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.

This is a Field Based position with a Home-Based office.

Areas:Dallas SDA

If you reside within 100 miles of Dallas, TX you will have the flexibility to work a Hybrid/Field schedule as you take on some tough challenges.

Primary Responsibilities:

RequiredQualifications:
  • Must possess one of the following:
    • High School Diploma/GED (or higher) AND 5+ years of experience working with ABD/SSI population within a clinical health care role
    • Associate's degree with 3+ years of clinical experience
    • Unrestricted LPN/LVN license in the state of TX with 3+ years of clinical experience
    • CHW certification with 3+ years of clinical experience

  • Access to reliable transportation and the ability to travel up to 75% of the time in this 'assigned area' to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
  • High speed internet in place of residence
  • Must be willing to use approved assessment technologies, including AI-assisted tools, to perform and document functional assessments
Preferred Qualifications:
  • LVN/LPN License in state of Texas
  • Ability to create, edit, save and send documents, spreadsheets and emails
  • Reside within commutable distance of assigned duties
  • Works on Service Plan

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $24.00 to $43.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
