Job summary

TRU Community Care in Colorado is seeking a Hospice Medical Social Worker to provide psychosocial services and support to patients and families in home and facility settings. The role involves working within the Interdisciplinary Team to coordinate care and participate in care planning.

The position requires an MSW and experience in end-of-life care preferred, with CPR cert or ability to obtain within 60 days, and a valid driver’s license for community visits.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• 1 year of care setting experience in end-of-life care preferred.
• CPR certification or ability to obtain within 60 days of hire.
• Must pass agency background checks and drug screening.
• Must possess a valid driver’s license and reliable transportation.

Responsibilities

• Provides psychosocial services for the team caseload and patients/families.
• Delivers direct psychosocial services to patients and families.
• Collaborates with IDT members and other health care providers.
• Attends patient care conferences in LTC/AL facilities as needed.
