Job summary

Regional Hospice is seeking an RN to serve our Springfield, Missouri region. The role involves traveling to patients’ homes, providing comprehensive assessments, symptom management, and emotional support.

The RN Case Manager will coordinate care with LPNs, CNAs, and the interdisciplinary team. The ideal candidate is currently licensed in Missouri, lives within a commutable distance to Springfield, and is compassionate, team-oriented, and proficient with EMR charting.

Qualifications

• Must be currently licensed in Missouri as an RN.
• Live within commutable distance to the Springfield office.
• Willing to train and possess a gentle, loving heart; teamwork.
• Proficient with EMR charting and mileage computation apps.

Responsibilities

• See patients in private homes, assisted living, skilled nursing facilities, or any patient’s home within a set area.
• Perform head-to-toe assessments and provide symptom management and skilled care.
• Supervise LPNs and CNAs; monitor medications and needs; coordinate DME and wound care.
