Job summary

Regional Hospice in Joplin, MO, is seeking a Full Time RN Case Manager to serve patients in the Joplin area. The RN will conduct in-home visits, assess patients, and provide symptom management for terminal illnesses.

The role offers autonomy, field-based work with some office meetings, and collaboration with a caring team. A Missouri RN license and reliable transportation are required.

Qualifications

• Must be a Missouri-licensed RN and reside within commutable distance to the Joplin office.
• Ability to train and work as part of a supportive team.
• Proficient in EMR charting and using mobile/email apps for mileage tracking.

Responsibilities

• Conduct head-to-toe assessments and provide symptom management for patients with terminal illnesses.
• Supervise LPNs and CNAs, monitor medications, and coordinate care.
• Deliver emotional support and education to patients and families.
