Job summary

Regional Hospice is seeking a Full Time RN Case Manager to serve patients in the Lebanon, MO area. The role involves driving to private homes, assisted living facilities and SNFs, conducting head-to-toe assessments, managing symptoms and providing education on death and dying while offering emotional support.

You will supervise LPNs and CNAs, review medications, assess DME needs and provide wound care as needed.

Qualifications

• Must hold an active Missouri RN license and live within commutable distance.
• Willing to train; compassionate, supportive for patients and families facing terminal illness.
• Proficient with EMR charting and mileage tracking apps.
• Office hours Mon–Fri, 8am–5pm; reliable transportation and car insurance required.

Responsibilities

• See patients in private homes, assisted living facilities, SNFs, or any home setting.
• Perform head-to-toe assessments, manage symptoms, and provide education on death and dying.
• Supervise LPNs and CNAs; monitor medications and DME needs; provide wound care when needed.
