Job summary

Qualifications

• Registered Nurse license in the practice state.
• BLS for Healthcare Providers certification (AHA) within the required period.
• Associate's Degree in Nursing.
• Typically 2 years of nursing experience.

Responsibilities

• Performs scheduled visits and collaborates with the interdisciplinary team.
• Assesses physical, psychosocial and environmental needs; updates care plan.
• Communicates plan changes to physicians and the primary RN.
• Provides ongoing assessment of patient response and revises care plan.
• Schedules patients and case conferences with the primary RN.
• Assists with discharge planning and uses resources for continuity of care.
• Documents and updates patient records per guidelines.
• Adheres to published policies and patient safety standards.
