Job summary

VitalCaring Group in Jensen Beach, FL is seeking an experienced Home Health RN to lead patient care in patients' homes. You will manage the plan of care, coordinate across disciplines, and drive outcomes from admission through discharge.

This field-based role offers clinical autonomy, complex high-impact care, and a supportive team. You will build strong patient relationships, educate families, and document care in the EMR.

Qualifications

• Active RN license and CPR certification required.
• BSN preferred; equivalent nursing degree considered.
• Minimum one year RN experience (home health, med-surg, ICU or acute care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems and documentation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including meds management, treatments, and interventions.
• Serve as case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and adjust care based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
