Job summary

VitalCaring is seeking a Field-Based Home Health Registered Nurse in Florida to lead patient care in home settings. You will manage plans of care, coordinate across disciplines, and drive outcomes from admission to discharge.

You'll perform comprehensive in-home assessments, develop individualized plans with physicians, deliver skilled nursing care, monitor progress, and educate patients and families to support independence.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
