Job summary

VitalCaring Group is seeking a Field-based Home Health RN in Oklahoma. You will serve as the clinical lead for your patients, coordinating across disciplines and driving outcomes from admission through discharge.

You’ll perform in-home assessments, develop individualized care plans, deliver skilled nursing care, and educate families to support independence. A strong clinical judgment and autonomous yet collaborative work style are essential.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered care.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and adjust decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
