Job summary

VitalCaring Group is seeking a Registered Nurse (RN) for field-based Home Health in Oklahoma. You will lead the plan of care, coordinate across disciplines, and drive outcomes from admission through discharge.

Qualifications

• Active RN license in Oklahoma and CPR certification.
• Graduate from an accredited nursing program; BSN preferred.
• Proficiency with EMR systems and care coordination.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including medications and treatments.
• Serve as case manager coordinating therapists, aides, and physicians.
• Monitor progress and adjust care based on changing conditions.
• Educate patients and families on disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise Home Health Aides and participate in on-call rotations.
