Graham Healthcare Group is seeking a Home Health Occupational Therapist in Somerset, PA to deliver patient-centered care within a team-based environment.

The role offers 40 hours per week, 8-hour shifts, and a staff employment arrangement with competitive pay of $80,000 to $100,000 per year, plus a $10,000 first-year success bonus and a $5,000 sign-on bonus. Comprehensive benefits include 401(k) matching, holiday leave, and more.

Formación

• Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Licensed Occupational Therapist in the state of practice.
• Minimum of one year healthcare experience.
• Valid driver’s license with ~20% travel to patient locations.
• Ability to meet charting deadlines and manage time effectively.
• Experience with EMR software and mobile devices.

Responsabilidades

• Provide management and delivery of patient care plans in home health.
• Collaborate with AHN Healthcare@Home and care teams to support patient outcomes.
• Support flexible scheduling and document care using EMR systems.
