Job summary

Graham Healthcare Group is seeking a Home Health Occupational Therapist for a role in Harwick, Pennsylvania. This full-time position offers $80K-$100K per year, 40 hours weekly, and a supportive team-based environment within AHN Healthcare@Home.

You will manage and deliver patient care plans, collaborate with the care team, and travel to patient locations. Requires OT license, BS/MS in OT, 1 year healthcare experience, and EMR proficiency.

Qualifications

• Bachelor or Master of Science in Occupational Therapy with ACOTE accreditation.
• Licensed OT in the state where you work.
• Minimum one year of healthcare experience.
• Valid driver’s license and ability to travel ~20% of the day.
• Strong time-management and charting accuracy.
• Experience with tablets, mobile devices, and EMR software.

Responsibilities

• Manage and deliver patient care plans in home health.
• Collaborate with the AHN Healthcare@Home team to coordinate care.
• Document clinical progress and outcomes in the EMR.
• Support scheduling and patient education.
