Job summary

Elevance Health is seeking a Behavioral Health/ Foster Care-focused Managed Care Coordinator. This field-based role requires travel to client sites within specified Virginia locations and possibly other areas.

Responsibilities include case management, needs assessments, and coordinating with clinical teams to address member needs. Minimum requirements include a BA/BS and 1 year of related experience; QMHP/MSW/BSW preferred.

Qualifications

• BA/BS degree or equivalent required.
• Minimum of 1 year of experience working directly with people related to the specific program population or related community-based organizations.
• QMHP, MSW or BSW degree preferred; state law and contract requirements may dictate exact qualifications.
• Willingness to travel to worksite and other locations as necessary; Foster Care and behavioral/mental health experience preferred.
• LTSS / DD waiver experience preferred.

Responsibilities

• Perform telephonic or face-to-face history and needs assessments using predefined questions.
• Identify members with potential clinical health care needs and coordinate with clinical teams; facilitation does not involve clinical judgment.
• Manage non-clinical needs of members to ensure efficient utilization of services.
• Establish short and long term goals with the member and caregivers.
• Identify members who may benefit from expanded services.
