Position Overview:
The ECF CHOICES Support Coordinator manages members enrolled in the Employment and Community First CHOICES program, focusing on supporting members with intellectual or developmental disabilities. Responsibilities include addressing medical, behavioral, and socioeconomic needs to promote appropriate service utilization and improve care quality.
This is a home-based role with field responsibilities approximately 50-75% of the time, traveling within Sumner, Wilson, Cannon, and Davidson counties. Remote work is flexible for Tennessee residents.

The hourly pay range is $28.61 to $56.06, depending on factors such as location, education, and experience. Benefits and incentives are available subject to eligibility.
