Position Overview:

The ECF CHOICES Support Coordinator manages members enrolled in the Employment and Community First CHOICES program, focusing on supporting members with intellectual or developmental disabilities. Responsibilities include addressing medical, behavioral, and socioeconomic needs to promote appropriate service utilization and improve care quality.

This is a home-based role with field responsibilities approximately 50-75% of the time, traveling within Sumner, Wilson, Cannon, and Davidson counties. Remote work is flexible for Tennessee residents.

Primary Responsibilities:

  1. Conduct face-to-face or telephonic assessments of members’ medical, behavioral, functional, cultural, and social needs.
  2. Develop and implement person-centered care plans to manage health conditions, promote wellness, and address social determinants of health.
  3. Create individualized care plans with goals and interventions aligned with the member’s readiness to change.
  4. Collaborate with care teams, providers, and community resources to execute care plans.
  5. Provide referrals and linkages to internal and community-based services, including housing, pharmacy, peer support, and healthcare providers.

Required Qualifications:

  • Degree in health, human services, or behavioral health (e.g., Social Work, Nursing, Psychology, Counseling) plus 3+ years of experience, or current unrestricted licensure as a Social Worker, or LPN with 5+ years of experience.
  • At least 2 years of experience serving individuals with disabilities.
  • At least 1 year of experience working with community providers and addressing social determinants of health.
  • Proficiency with MS Office and reliable transportation for local travel.

Preferred Qualifications:

  • Community case management experience for individuals with complex needs.
  • Training or certification in motivational interviewing, trauma-informed care, or person-centered care.
  • Experience with team-based care and co-occurring disorders.
  • Background in managed care.

The hourly pay range is $28.61 to $56.06, depending on factors such as location, education, and experience. Benefits and incentives are available subject to eligibility.
