Qualifications

• At least one year of clinical experience working with elderly or disabled populations.
• Demonstrated knowledge of medical terminology required.
• Bilingual in English and secondary language preferred.

Responsibilities

• Conduct on-site visits with each client and caregiver.
• Evaluate clients and formulate individualized plans of care.
• Coordinate the social services delivery for all program participants.
