As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. Interesting in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role.

This position is full-time (40 hours/week) Monday – Friday, normal business hours between 8:00am to 5:00pm. It may be necessary, given the business need, to work occasional overtime. This position is a combination of telephonic and field-based with a home-based office. You will work from home when not in the field.

Location: Ocean, Burlington, Camden, Gloucester, Monmouth and surrounding cities, NJ

If you are located within near Ocean, Burlington, Camden, Gloucester, Monmouth, or other surrounding cities in New Jersey, you will have the flexibility to work remotely* as you take on some tough challenges.

Local travel up to 50% and mileage is reimbursed at current government rate.

This is a Field Based role with a Home-Based office.

Primary Responsibilities

  • Create a positive experience and relationship with the member
  • Help member set goals and develop a care plan to achieve those goals
  • Proactively engage the member to manage their own health and healthcare
  • Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician
  • As needed, help the member engage with mental health and substance use treatment
  • Support the member to ensure pick-up of their prescriptions
  • Provide member education on community resources and benefits
  • Conduct post discharge activities from hospital facility and support connection to social services
  • Support member to engage in work or volunteer activities, if desired, and develop stronger social supports through deeper connections with friends, family, and their community
  • Partner with care team (community, providers, internal staff)
  • Knowledge and continued learning of community cultures and values

Required Qualifications

  • High School Diploma/GED (or higher)
  • 1+ years of field-based experience
  • 1+ years of experience with knowledge of the resources available, culture and values in the community
  • Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and the ability to use multiple web applications
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Ability to travel locally up to 80 miles round trip and up to 50% of the time
  • Reside within a commutable distance to Ocean, Burlington, Camden, Gloucester, Monmouth and surrounding cities in NJ
  • Access to reliable transportation with current automobile insurance that will enable you to travel to client and / or patient sites within a designated area
  • Valid US driver’s license

Preferred Qualifications

  • Associate’s Degree (or higher) in a health-related field
  • 4+ years of equivalent community outreach work experience
  • LPN (Licensed Practical Nurse) Licensure or CNA / HHA
  • Community Health Worker (CHW) Accreditation
  • Experience working in Managed Care
  • Knowledge of Medicaid/Medicare population

  • Demonstrated ability to work well with others
  • Strong organizational and time management skills
  • Ability to work in a fluid environment

The salary range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
