Schedule and Travel

This full-time position (40 hours/week) is Monday – Friday, with flexible 8-hour shifts between 8am – 5pm EST. Occasional overtime and up to 50% local travel may be required. The role is primarily telephonic from home, with community outreach activities as needed. Ohio residents may telecommute.

Primary Responsibilities

• Build positive relationships and experiences with members
• Support members to avoid hospitalization through regular primary care visits
• Engage members actively with their healthcare providers
• Assist with medication pick-up and adherence
• Proactively support care management and transitions
• Provide education and promote compliance with care plans
• Partner with care teams and understand community resources
• Conduct member assessments and learn community cultures

Qualifications

Required

• High School Diploma/GED
• Ability to work remotely with high-speed internet and a dedicated workspace
• Knowledge of community resources and cultures
• Reside in Ohio
• Availability during standard business hours and ability to travel locally
• Reliable transportation and valid driver’s license

Preferred

• Bachelor’s degree in social work or healthcare admin
• Community Health Worker accreditation
• Experience in managed care, behavioral health, or substance abuse
• Knowledge of Ohio Medicaid & Medicare
• Bilingual in English and Spanish

Compensation

Hourly range: $19.86 - $38.85, based on experience and market factors. Benefits include a comprehensive package, incentives, and stock options.
