Centene Corporation is seeking a community health professional to outreach to Medicaid/Medicare members, identify care gaps, and connect them with services. You will conduct home visits, assess needs, and help members navigate care plans and benefits.

Collaboration with providers to share data and support quality improvement is essential. You will educate members on preventive care, document interactions, and work with internal teams to improve outcomes.

Competenze

• Bachelor's degree (Social Work, Public Health, Nursing, or related field) or equivalent experience.
• 2+ years in community health or healthcare quality.
• Experience working with health plan members and navigating community resources.
• Familiarity with Medicaid/Medicare programs and quality measures (e.g., HEDIS).
• Strong communication and interpersonal skills.
