Job summary

Firefly Children & Family Alliance is seeking a Home-based Case Manager to serve families in Plainfield and surrounding counties. The role involves delivering in-home services, education, transportation, and court attendance to support family goals in Putnam, Hendricks, and Morgan counties.

You will work in the field with families, coordinate with schools and agencies, and respond to crises while maintaining confidentiality and cultural sensitivity.

Qualifications

• Bachelor's degree in social work or related field required.

Responsibilities

• Monitor and coach family progress during home visits.
• Create service plans with goals for each youth/family.
• Conduct intake and needs assessments for families.
• Prepare written reports for relevant child services and probation offices.
