Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking a field-based Case Manager to assess and coordinate care for members, ensuring services are delivered efficiently and in line with program guidelines.

You will collaborate with members, families, physicians, and care teams to plan, implement, and evaluate care, while maintaining comprehensive documentation and meeting quality standards. Local travel within New Jersey is required.

Qualifications

• Bachelor degree in health-related field.
• 3–4 years broad clinical experience.
• Minimum 2 years in managed care industry.
• NJ SW license preferred; local travel required.

Responsibilities

• Assist with implementation and facilitation of services to members.
• Coordinate delivery of high-quality, cost-effective care.
• Collaborate with member/family, physician and healthcare team.
• Document accurately according to standards of practice and policies.
• Field-based and/or telephonic case management activities.
• Participate in quality improvement initiatives.
