Job summary

MetroPlusHealth is seeking a Field-based HARP Care Manager in New York. You will develop and communicate comprehensive care plans, collaborate with Health Home teams, physicians, and behavioral health providers, and facilitate post-discharge follow-up for members with complex health needs.

Responsibilities include in-home assessments, coordination of services across settings, and ensuring timely aftercare appointments.

Qualifications

• Master’s degree in Behavioral Health or related field (required).
• 3-5 years Behavioral Health and/or case management experience in managed care or inpatient/outpatient settings.
• NY state licensure preferred (LMHC/LMFT/LCSW/LMSW) and strong communication skills.

Responsibilities

• Clinically assess members for acute behavioral and physical health needs.
• Coordinate care plans with Health Home, physicians, and BH/SUD providers.
• Schedule aftercare follow-ups and ensure appointments are in place.
• Conduct home visits and in-community assessments as needed.
• Review and approve Health Home Plans of Care and CORE/HCBS requests.
• Document care coordination activities in EMR systems and ensure regulatory compliance.
