Job summary

MetroPlusHealth is seeking a HARP Care Manager to develop, facilitate, and communicate comprehensive care plans in partnership with members, Health Homes, and providers. The role focuses on assessment, planning, and advocacy for complex health needs through in-field community outreach.

The incumbent schedules aftercare follow-ups, ensures appointments are in place, and coordinates with inpatient teams to secure appropriate post-discharge care.

Qualifications

• Master’s Degree required.
• 3–5 years of Behavioral Health experience in managed care or psychiatric hospital settings.
• 1-year HARP experience preferred.
• Field-based Case Management within Integrated Collaborative Care Model.
• Experience with chronic condition management (e.g., Diabetes, HIV, Heart Disease).
• Bilingual preferred.

Responsibilities

• Clinically assess members for acute behavioral and physical health needs and generate referrals.
• Conduct Utilization Review for designated Case Management members.
• Meet with members in homes, shelters, community settings, and hospitals.
• Conduct initial and ongoing Behavioral Health clinical assessments.
• Develop formal plans of care and notify physicians of changes in care plans.
• Assist with coordinating post-discharge aftercare appointments and follow-ups.
• Oversee care coordination and documentation in EMR systems.
• Educate providers on treatment compliance and promote long-acting injectables.
• Participate in Interdisciplinary Care Team meetings and quality improvement projects.
