Job summary

Elevance Health is seeking a Managed Care Coordinator - Behavioral Health/Foster Care to oversee members\' individual service plans, engaging with families and providers. The role operates in the field, traveling to client sites as needed, with occasional meetings at offices. The position requires a BA/BS and at least 1 year of related community-based experience.

Travel to worksites is expected. Preferred QMHP/MSW/BSW background, Foster Care and behavioral health experience are advantageous.

Qualifications

• BA/BS degree required.
• Minimum of 1 year of experience working directly with people related to the program population or equivalent background.

Responsibilities

• Perform telephonic or face-to-face history and program needs assessments.
• Identify members with potential clinical health care needs and coordinate with the clinical team.
• Manage non-clinical needs of members to ensure cost effective service utilization.
• Establish short and long term goals with members, caregivers, and physicians.
• Identify members who would benefit from expanded services.
