Job summary

Healthfirst, New York City's largest not-for-profit health insurer, seeks an UAS Nurse Assessor to conduct home and virtual assessments for prospective and current members across Brooklyn and nearby areas.

You will navigate multiple care platforms and support enrollment determinations, with a focus on value-based care and member independence. Opportunities for growth in a supportive environment. Training provided and a stable benefits package included.

Qualifications

• New York State RN license required.
• Ability to travel around downstate NY including NYC.
• Proven tech proficiency across multiple platforms.
• Moderate MS Word, Excel, and Outlook skills.
• PEDS: 1 year pediatric field experience (preferred).

Responsibilities

• Conducts home and virtual assessments using UAS; determine enrollment and eligibility.
• Navigates UAS software, EMR, TruCare to document assessments.
• Assesses service needs to determine care plans and necessary adjustments.
• Supports person-centered care planning through in-person visits.
• Liaises between member and care team and provides feedback to agencies.
• Offers community-based training for frail populations.
