In this role, you'll work in the field and coordinate the long-term care needs for patients in the local community, supported by an elite and dynamic team. In this Field RN role, you will be an essential part of an Integrated Care Model by relaying pertinent information about member needs and advocating.

If located in New York City, NY, you will have the flexibility to work remotely* while traveling within the Bronx and/or Westchester County.

Primary Responsibilities:
  • Assess, plan, and implement care strategies individualized to patients, aimed at the most appropriate, least restrictive level of care
  • Utilize the NYS UAS Assessment in the home or via telehealth to develop a person-centered service plan
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Communicate health-related information to all stakeholders to ensure quality, coordinated care is provided expeditiously
  • Advocate for patients and families to ensure their needs and choices are supported by the healthcare team

Qualifications -
Required Qualifications:
  • Current, unrestricted RN license
  • 2+ years of clinical work experience
  • 1+ years of community case management experience
  • Knowledge of home health, hospice, public health, or assisted living
  • Proficiency with MS Word, Excel, Outlook
  • Willingness or ability to travel within the assigned region
  • NY state ID or ability to obtain one prior to hire
Preferred Qualifications:
  • Behavioral health or clinical degree
  • Experience with electronic charting
  • Experience arranging community resources
  • Field-based work experience
  • Background in managing populations with complex needs
  • Proficiency in UASNY

New York Residents Only: The hourly range is $28.03 to $54.95, based on experience and qualifications. Benefits include a comprehensive package, incentives, stock purchase, and 401(k).
