Job description

Location: This field-based role involves traveling to client sites or designated locations, with occasional office attendance for meetings or training. The candidate will support a territory around Gaitherburg and Bethesda, MD, and may also cover NW DC.

The Field Advanced Practice Provider collaborates with physicians, other healthcare providers, and families to develop complex care plans aligned with patient goals and values. Provides clinical and non-clinical support to palliative care patients within a designated territory.

How you'll make a difference:

  1. Provides home-based care to patients with chronic and complex conditions, developing and implementing clinical plans of care.
  2. Gathers patient history, conducts physical exams, and develops treatment plans based on patient goals and current health status.
  3. Addresses physical and psychosocial needs of patients and families with support from an interdisciplinary team.
  4. Educates patients and families on medication use, side effects, illness progression, diet, and crisis prevention.
  5. Coordinates care with other clinical team members and healthcare providers to ensure timely and effective treatment.
  6. Maintains accurate medical records and documentation in accordance with regulations and policies.
  7. Participates in ongoing education as required by state regulations.
  8. Prescribes medications as permitted by state law.

Minimum Requirements:

  • MS in Nursing
  • Current unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in applicable state(s)
  • DEA registration or eligibility
  • CPR certification

Preferred skills, qualifications, and experiences:

  • At least 2 years in home-based, complex care, hospice, or palliative care
  • Experience in hospital, oncology, ER, or ICU settings is highly preferred
  • Experience with Electronic Medical Records (EMR)
  • Bilingual in English/Spanish is preferred
  • Completion of Tuberculosis test for Carelon Health
  • Travel may be required

Salary range: $113,064 to $169,596 for candidates in Maryland. Benefits include comprehensive packages, incentives, stock options, 401(k), and more. The salary is based on various factors, and actual compensation may vary.
