Elevance Health is seeking a Field Advanced Practice Provider to operate in the field and travel to client sites within commuting distance of Baltimore, MD, with occasional office attendance for meetings or training.

The position requires MS in Nursing, an unrestricted NP/PA license, DEA registration or eligibility, and CPR certification; travel and TB testing for Carelon Health are noted. This is a patient‑focused role delivering high‑quality palliative care in home settings.

Field Advanced Practice Provider

Job Overview

Field‑based role primarily operating in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Candidates must reside within commuting distance of Baltimore, MD.

Responsibilities
  • Collaborate with company physicians, other providers, and family members to develop complex care plans in accordance with the patient’s health status and goals.
  • Provide clinical and non‑clinical support to patients in palliative care within a designated territory.
  • Deliver services in home settings to care for adult patients facing chronic and complex conditions such as co‑morbid medical and mental health diagnoses.
  • Gather history, perform physical examinations and diagnostics, and develop treatment plans based on patient goals of care and current conditions.
  • Meet patients’ and families’ physical and psychosocial needs with support from the interdisciplinary team.
  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention.
  • Coordinate care with other clinical team members, patients’ other physicians and medical providers to ensure timely, integrated care consistent with state regulations and company policy.
  • Maintain patient medical records and documentation in compliance with state regulations and company standards.
  • Participate in continuing education as required by state.
  • Prescribe medication as permitted by state prescribing authority.
Required Qualifications
  • MS in Nursing.
  • Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s).
  • DEA registration or eligibility.
  • CPR certification.
  • Travel may be required.
  • For the Carelon Health business unit, satisfactory completion of a Tuberculosis test is required.
Preferred Experience
  • 2+ years in home‑based services, complex care, hospice and/or palliative care.
  • Experience in bedside hospital, oncology, emergency department, or intensive care unit settings.
  • Experience with Electronic Medical Records (EMR).
  • Bilingual in English/Spanish.
Compensation

Salary range: $115,584 to $173,376 (Maryland).
