Job summary

RiseBoro Community Partnership seeks a Case Manager within its Functional Family Therapy-Adult program. You will collaborate with Therapists to provide collaborative case management for a caseload of 18–24 clients and facilitate ART groups for young adults aged 19–27.

You will participate in FFT-A training, maintain detailed notes, and coordinate with providers to support comprehensive, culturally responsive care aligned with FFT-A principles.

Qualifications

• Bachelor's degree required or equivalent professional experience (at least 4 years).
• Direct clinical experience with adolescents, adults, or family systems preferred.
• Experience in mental health settings with youth/young adults is advantageous.

Responsibilities

• Work with Therapists to provide therapeutic case management for 18–24 clients.
• Facilitate ART groups for young adults 19–27.
• Engage in FFT-A training, consultations, and peer supervision.
• Maintain FFT-A case notes, statistics, and outcome data.
• Coordinate services with other providers and document case narratives and plans.
• Assist with development and implementation of individualized service plans.
