Job summary

Wheeler Clinic in Hartford is seeking a Crisis Clinician to provide crisis assessment, intervention and stabilization for children birth through age 17, delivering direct services and coordinating with families, schools and community partners in a collaborative team-based model.

You will conduct screenings, formulate diagnoses per DSM, develop individualized crisis and treatment plans, and arrange safe transitions.

Qualifications

  • Master’s degree in a relevant mental health field (MFT, MSW, Counseling) or Doctoral degree in Psychology.

Responsibilities

  • Conducts program specific screening and evaluation and documents DSM-based formulations and treatment plans.
  • Provides mobile service in community settings (homes, schools, hospitals, etc.).
  • Coordinates with families, schools and external providers to ensure crisis stabilization and safe transitions.
  • Works within a team-based care model and adheres to program expectations.

Education

Master’s degree in Marriage and Family Therapy, Social Work, or Counseling

Position Summary

The Crisis Clinician provides crisis assessment, intervention and stabilization clinical and case management services for children birth through age 17. The clinician will foster positive working relationships with clients, families, providers, educators and outside key stakeholders and utilize a strength based-solution focused model of care. The Crisis Clinician will utilize a team approach.

Education And Experience/Qualifications

Master’s degree in Marriage and Family Therapy, Social Work, or Counseling, or Doctoral degree in Psychology. Valid Driver’s License is required. Safely operates vehicle in the community, based on program needs.

Licensure Requirements For Clinical Positions

Must hold one of the following credentials: LMSW, LCSW, LMFTA, LMFT, LPCA, LPC or licensed or license-eligible Psychologist in the State of Connecticut. Please note that you can apply prior to receiving the above credentials.

Essential Duties And Responsibilities

Conducts program specific screening and evaluation, and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents, clear clinical formulations, crisis and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant.

Demonstrates knowledge of psychopathology, crisis theory and other behavioral health concerns and primary modalities of psychotherapeutic intervention. Triages clinical emergencies and implements program specific safety and consultation protocols. Utilizes best practices and evidenced based practice models where appropriate and adheres to model and program expectations.

Provides mobile service in community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program.

Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.

Communicates in a positive, effective manner with client, family, and all internal and external providers. Attends any required meetings.

May work with emergency departments, inpatient units and congregate care providers to identify and coordinate crisis and diversionary planning for high needs youth in need of intensive community-based planning to prevent multiple admissions to such settings and otherwise promote the use of MCIS to coordinate intensive plans for high needs youth.

May participate in community based meetings and conference calls with emergency departments, inpatient units, congregate care providers, other MCIS providers and the Department of Children and Families to assist in care planning and to address system issues which may impact increased emergency department and inpatient unit utilization of high needs youth.

With assistance from Program Manager and other leadership, may coordinate with other MCIS team members to provide intensive follow-up care and stabilization to identify high needs youth.

Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols.

Documents all client related information in compliance with agency policy and contractor requirements.

Attends all MCIS trainings as required by the MCIS Performance Improvement Center.

Attends and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory directives.

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
