Job summary

Rising Ground in Brooklyn is seeking a qualified candidate to provide direct services to children and families, assess needs, implement treatment plans, and support goals across home, clinic, and community settings.

The role requires a BA/BS with 2+ years’ experience, strong communication, computer skills, and ability to work in person. Bilingual English/Spanish is a plus, with weekday hours Monday through Friday.

Qualifications

• BA/BS degree and at least 2 years’ experience working with children and families.
• Excellent verbal and written communication skills.
• Computer proficiency; knowledge of CONNECTIONS a plus.
• Bilingual (English/Spanish) is a plus.

Responsibilities

• Provide direct services to children and assess needs for therapeutic services.
• Attend FTC and related case conferences for children needing clinical support.
• Assist parents and children with treatment plans.
• Make home visits and collateral contacts to meet ACS and OCFS requirements.
• Provide in-home and in-community behavior management and developmental support.
• Support behavioral goals developed by the treatment team.
• Create daily experiences to promote self-esteem, socialization, and family unification.
• Perform child/youth care tasks including ADL and crisis planning.
• Document progress notes in CONNECTIONS.
• Other duties as assigned by supervisor.

Position Type/Expected Hours Of Work

This is a full-time position. Days and hours of work are Monday through Friday in Brooklyn. Work hours options are 9:00 am - 5:00 pm, 9:30 am - 5:30 pm, 10 am - 6 pm.

Additional Requirements

• Authorized to work in the U.S.
• Ability to work in-person in Brooklyn for at least 3 days or more per week.
• Ability to travel to other Rising Ground sites (if required)
