About the role

Her Serenity Fertility Care is a growing integrative fertility practice dedicated to helping women navigate their fertility journey with personalized, compassionate care. We are looking for an experienced, empathetic, and bilingual (English/Spanish) Fertility Care Case Manager to join our team and serve as a key point of contact for our patients from the moment they reach out through every step of their care.

What you’ll do

• Manage patient onboarding, intake coordination, and ongoing case follow-up.
• Serve as the primary point of contact for fertility patients — in both English and Spanish — answering questions and providing support throughout their care.
• Coordinate lab orders, referrals, and follow-up appointments with clinical and specialist teams.
• Maintain accurate patient records and documentation in the EMR system.
• Communicate care plans clearly to patients in their preferred language, ensuring they understand next steps and feel fully supported.
• Assist with scheduling, prior authorizations, and insurance-related inquiries as needed.
• Work closely with the clinical team to ensure seamless continuity of care.

What we’re looking for

• Fluent in English and Spanish — required. You will be working directly with Spanish‑speaking patients on a daily basis.
• Minimum 2 years of experience in GYN and/or fertility care coordination or case management — this is a must.
• Strong understanding of fertility diagnostics, treatments, and patient support needs.
• Excellent communication and interpersonal skills — you genuinely connect with patients.
• Experience with EMR systems and patient documentation.
• Highly organized, detail‑oriented, and able to manage multiple patients simultaneously.
