The New Jewish Home Certified Home Health Agency (CHHA) provides home-based care and rehabilitation therapy for patients recovering from an acute episode or other adverse health event. The New Jewish Home has long experience delivering rehabilitation and home care services. Our goal is to help each patient get well, stay well, and return to health and independence as quickly as possible.

Job Summary: The CHHA is seeking Fee-For-Service Social Workers to provide and coordinate multidisciplinary health services for patients in the community. They will conduct assessments and collaborate with physicians and other team members on the implementation of care plans with the goal of discharging clients from the program.

Further duties include ensuring that all documentation required is completed within regulatory guidelines, including Physician Orders (485) and OASIS, which must accurately reflect the clinical and functional status of the patient to follow reimbursement policies.

Job Qualifications:

• Valid NYS Social Work License
• Graduate from an accredited school of Social Work
• Ability to travel to areas designated by the region served
• Availability at least one weekend per month
• Own a car for all non-Manhattan regions
• Homecare experience required*
• On-call availability required
• Willingness to travel to NYC metro area regions and Westchester territories
• Availability to work every other weekend and at least 3 holidays a year
