Job summary

Sedgwick County seeks a dedicated professional to provide direct in-home and community-based parenting education and support to fathers and families, focusing on prenatal, postnatal, and interconception periods.

You will conduct home visits, screen for psychosocial risks, and connect families with needed services while documenting interactions in the EHR within 48 hours and coordinating group activities for father involvement.

Qualifications

• Associate degree or two years of college in Social Work, Health Education, or related field.
• Must complete FEMA IS-100 and IS-700 courses within 30 days of employment.
• Driving level position per Sedgwick County policy requires a valid Kansas driver’s license without restrictions and current proof of automobile insurance.

Responsibilities

• Conducts home visits and screens fathers to evaluate psychosocial risk factors.
• Provides referrals and education on prenatal/postnatal topics.
• Documents risk factors and client interactions in EHR within 48 hours.
• Facilitates fatherhood/male partner support groups and conducts community outreach.
• Represents Healthy Babies Fatherhood Program at health fairs, coalitions, and community events.
