Fast Track Home- Engagement Specialist

Status: Full-time (37.5 hours/week)

Pay Range: $33,100.00 - $41,750.00

Work Location: Remote except for when participating in required in-person meetings or training sessions.

Job Summary: Responsible for telephone screening and assessment of individuals to determine program eligibility for appropriate care options. Provide telephone support that increases client engagement in COA programs.

Essential Functions:

  • Engaging, assisting, educating and supporting callers from the COA 5 county area who are attempting to navigate the Care Transitions network.
  • Conducting cold calls to prospective clients explaining the program and promoting services.
    • Providing information on community resources and services.
    • Providing callers with non-biased information to assist the caller in coping with transitional care options/decisions.
    • Completing telephone and screening from all referral sources, to determine an individual’s appropriateness for transitional care program and complete referrals to Care Transitions program and community agencies as appropriate.
    • Scheduling in person visits as appropriate, appointments with members and/or caregivers , following policy with regard to timeframes, clustering appointments and monitoring volumes.
    • Working closely with COA staff to communicate client related issues identified through conversation with clients or referral sources.
    • Assessing Fast Track Home (FTH) eligibility, maintain contact with FTH clients through phone calls for the duration of the 60-day intervention.


    Experience:

    • At least one-year prior experience in health care, community outreach, community resources, medical social work, geriatrics, or providing individuals with information about options available to meet long-term care needs or with related information such as that which is provided through information and referral or assistance.
    • Sales or marketing experience a plus.
    • Good working knowledge and understanding of issues related to the elderly and disabled population.


    Skills & Abilities:

    • Tolerant, empathetic, good interviewing capability.
    • Ability to collaborate and problem solve in a team environment.
    • A good working knowledge and understanding of issues related to the older adult population.
    • Good oral and written communication skills
    • A valid driver’s license and reliable transportation.
    • Excellent computer skills including the Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed
