Qualifications

• Requires a 4-year degree in a human services field.
• One year of experience with developmental disabilities preferred.
• Knowledge of community resources and ability to facilitate positive change.
• Valid driver’s license with good driving record.

Responsibilities

• Provide support and resources to families and children with developmental disabilities.
• Meet clients in homes or other privacy-suitable locations.
• Participate in bi-weekly supervision and team meetings.
• Coordinate, educate and train families and supports on individual needs.
• Monitor progress toward goals and provide additional resources as needed.
• Advocate for culturally responsive care across systems.
