Working Title: FAMILY SUPPORT WORKER - 60022013

This is work providing support and basic social services to elderly and disabled individuals. Provides direct services to eligible Adult Protective Services clients. Performs a variety of paraprofessional duties in the office and in the community in support of Adult Protective Services staff, including transportation, home visits, and administrative functions in the following manner. Performs intake duties on a rotating basis. Assigns abuse reports to investigators and notifies investigators of report assignment. Maintains intake log. Creates, uploads, and sends correspondence to internal staff and to clients. Attends meetings and takes minutes. Generates monthly reports. Performs related administrative duties.. Assists counselors in maintaining a caseload of elderly and disabled adults by providing information and arranging for medical, housing, social, financial assistance, transportation, food resources and other adult services. Assists clients in completing forms for services and benefits, in arranging appointments and accessing community services. Maintains liaison with community agencies and service organizations in order to coordinate services rendered to individuals. Provides transportation for clients to court, placement facilities, for medical appointments, grocery shopping, paying bills and other client related errands. Completes and submits forms to update client files. Completes mileage and Leave and Attendance. Receives and sends email on computer and completes required written documentation. Completes all monthly HCDA vouchering, and TES vouchering as required. Performs travel, in private vehicle, in relation to the above duties, and to attend meetings, training programs, to pick up and deliver documents as assigned and as needed to assist the Adult Protective Investigators, Human Services Counselor III, and the management team. Performs other duties as assigned.
