Job summary

Hoyleton Youth and Family Services is seeking a Family Support Program Specialist to support youth and families in the Family Support Program (FSP). The role focuses on strengthening family stability and linking to community-based services across multiple counties in Illinois.

The position involves coordinating services, assisting with FSP applications, and connecting clients to crisis stabilization and clinical services as needed. CARE/TCI training is required after hire and ongoing.

Qualifications

  • Bachelor’s degree in social work, psychology, or related human service field.
  • Ability to participate in a fast-paced, multidisciplinary team environment.
  • Must possess and maintain valid driver’s license.
  • Must possess and maintain proof of automobile insurance.
  • Must be at least 21 years of age.

Responsibilities

  • Network with collaterals on behalf of active clients, including referral and linkage, and discharge planning with facilities.
  • Coordinate services youth may benefit from and link to community resources.
  • Assist families to access FSP covered services and other mental health services.
  • Identify behavioral health needs and connect youth to appropriate clinical services.
  • Maintain relationships with referral sources and partners to strengthen coordination.
  • Facilitate quarterly meetings with service providers and networks.
  • Maintain an accurate Community Resource Directory and ensure timely documentation.
  • Complete screenings, assessments, and other necessary clinical documentation.
  • Ensure compliance with COA and regulatory requirements.

Description
POSITION SUMMARY

The Family Support Program Specialist will be primarily responsible for providing services to youth enrolled in the Family Support Program (FSP). The goal of the FSP is to support eligible youth and their families by strengthening family stability, improving clinical outcomes, and promoting community-based services. Coverage areas include St Clair, Clinton, Randolph, Washington, Monroe and Perry Counties (DSA 2).

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
  • Responsible for networking with collaterals on behalf of active clients, including: referral and linkage, discharge planning with in-patient psychiatric facilities, and the advocacy of a client’s rights within the education, legal, state and mental health systems. This includes responsibility for coordinating services the youth may benefit from.
  • Responsible for working with families to access FSP covered services. Providing linkage to community mental health services, alternative community services, and residential treatment when it is determined to be medically necessary.
  • Available to help families with obtaining, completing and submitting the FSP application. Assisting parents in gathering information to complete the application process for the FSP program and answering questions about the process.
  • Responsible for working with families who are referred to the Specialized Family Support Program (SFSP) a 90-day program of crisis stabilization, community mental health, and assessment of services. This includes identifying behavioral health needs of youth at risk of custody of relinquishment, completing of an SFSP assessment report and to link those youth to the most appropriate clinical services.
  • Responsible for maintaining relationships with referral sources/ other partners to develop community connections.
  • Responsible for facilitation of quarterly meetings with all identified service providers and networks to strengthen relationships and coordination of services.
  • Responsible for maintaining an accurate Community Resource Directory
  • Communicate effectively within a team environment to meet the needs of those experiencing behavioral health crises
  • Complete screenings, assessments, and other necessary clinical documentation.
  • Maintain compliance with COA and other regulatory agency requirements.
  • Ensure that all documentation is completed timely and accurately.
  • Completes other duties as assigned.

EDUCATION AND/OR EXPERIENCE
  • Bachelor’s degree in social work, psychology, or related human service field.

  • Must possess and maintain valid driver’s license
  • Must possess and maintain proof of automobile insurance.
  • Must be at least 21 years of age
CARE & TCI

Hoyleton has adopted the CARE (Children and Residential Experience), developed by Cornell University, to guide our culture and practice. CARE is grounded in six core principles: Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed.

Completion of CARE and TCI training is an internal requirement for all staff and must be completed upon hire. Ongoing application of these principles is expected and essential to successful job performance.

TCI (Therapeutic Crisis Intervention).

Employees must:

  • Pass the written test annually
  • Pass the physical test every six months (if applicable to their role)

WORKING CONDITIONS

This position operates in a professional office, clinical, residential, community-based environment and may require travel to agency sites, client locations, partner organizations, based on role responsibilities.
The role involves periods of walking, standing, or sitting, and may occasionally require climbing stairs, bending, kneeling, or lifting materials up to 10 pounds depending on job duties.
Minimal exposure to environmental factors such as noise, dust, or varying indoor/outdoor temperatures may occur depending on program needs and client-related activities.
