Family Support Coordinator

The CBHC Family Support Coordinator (FSC) is an integral member of the interdisciplinary Youth/Young Adult Team, providing care coordination and case management support for youth in partnership with caregivers. The FSC assists families with navigating youth-serving systems, connecting and referring to services, and offering family support and advocacy. Care coordination is provided through the lens of lived experience as a caregiver to youth with behavioral health challenges.

In this role, the FSC will:

  1. Assist parents in becoming advocates for their children by participating in activities designed to address service plan health-related social needs.
  2. Attend school or community meetings alongside parents/caregivers as needed.
  3. Support parents/caregivers in accessing appropriate outside resources.
  4. Provide care coordination for youth with caregivers, navigating systems like DMH, DCF, schools/IEP services, and CBHI.
  5. Develop recommendations for community resources and supports beneficial to the family, in collaboration with the family and interdisciplinary team.
  6. Complete all required documentation accurately and promptly, complying with agency and state protocols.

Schedule: Full-Time, 40 hours per week

  • Weekend work is not required but is highly incentivized with a $5 weekend differential.

Pay: $22.27/hour

  • Additional $1 for CPS
  • Additional $2 for CARC
