Job summary

Massachusetts Department of Public Health seeks a Home Visitor for the F.O.R. Families Program in the DPIEC. The role includes assessing families, coordinating with health and human service providers, and offering referrals to stabilize housing and health outcomes.

Travel to shelters and a Canton site is required. Applicants should have a valid MA license, language skills in Creole or Spanish, and strong case-management capabilities in a fast-paced shelter setting.

Qualifications

• Minimum of three (3) years’ experience in the field.
• Bilingual in Haitian Creole or Spanish.
• Home visiting experience.
• Experience conducting intake assessments.
• Master’s Degree in social work, psychology, human services, or related field.
• Experience working with families experiencing homelessness.
• Experience using an electronic record.
• Experience working with computers and Windows-based applications.
• Ability to work under pressure and in a hotel/shelter setting.
• Proficient with Microsoft Office applications including Excel, Word, Teams, and Outlook.

Responsibilities

• Conduct comprehensive assessments of families experiencing homelessness to identify barriers to stability.
• Screen families for domestic violence, substance use disorders, and mental health issues, providing referrals and crisis management.
• Coordinate with DPH programs, state agencies, local governments, and community organizations to deliver services.
• Liaise with shelter providers and other agencies to address gaps in services.
• Develop and maintain a resources list for families and share with providers.
• Maintain organized client files and timely documentation.
• Input client data into EOHLC's electronic system within 5 days of visits.
