Compensation: $48,000 salary

Family Service Coordinators with NYAP work in the community as part of an integral service team. Under supervision, this role collaborates closely with youth, families, and community partners to provide advocacy and support, aiming for safe and successful community living. The position involves working with a wrap-around team and therapist to deliver crisis stabilization to families in need.

Responsibilities
  • Provide in-home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and interventions.
  • Travel daily to deliver community-based services to youth and families.
  • Coordinate and monitor services including mental health, medical, educational, psychological, vocational, and social services.
  • Build and maintain strong relationships with clients and families.
  • Conduct safety assessments, crisis response, and behavioral stabilization services.
  • Work under supervision of a therapist.
  • Participate in on-call duties.
  • Work evening hours as needed.
  • Conduct home visits.
  • Maintain a productivity rate of 40%.
Minimum Qualifications
  • Bachelor's degree in Social Work or Human Services from an accredited institution.
  • Preferably 2 years of direct service experience with youth and families.
  • Willingness to work flexible hours including afternoons and evenings.
  • Valid driver’s license, reliable transportation, insurance, and a clean driving record.
  • Proficient with computers, smartphones, tablets, and relevant software.
