Job summary

Childrens Home Society of Florida is seeking a Family Service Coordinator to assess and coordinate services for individuals with developmental disabilities. In this role, you will provide vital case management services, develop support plans, and conduct home visits.

Qualifications

• Bachelor's degree in Social, Behavioral or Health Sciences from an accredited university, required.
• Florida Driver’s License within 30 days from hire with daily access to a reliable vehicle, required.

Responsibilities

• Assess, secure, coordinate and monitor satisfaction of services.
• Develop support plans with outcomes and action steps.
• Provide case management services for program clients.
• Conduct home visits.
