Job summary

National Youth Advocate Program (NYAP) is seeking a Family Service Coordinator to work in the community, providing advocacy and support to youth and families under supervision. The role involves in-home services, transportation, counseling, and coordination with multiple service sectors to ensure safety and stability for youth.

Position requires flexibility for afternoon/evening hours, a valid driver's license, and proficiency with common software.

Qualifications

  • Degree in Social Work or comparable Human Services field from an accredited institution.
  • 2 years of work experience in direct service with youth and families strongly preferred.
  • Willingness to work flexible and non-traditional hours with afternoon and evening availability.
  • Must have a valid driver's license, reliable transportation, automotive insurance, and a good driving record.
  • Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet, and database programs.

Responsibilities

  • Provide in home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to youth and family.
  • Travel daily to provide community-based services to youth and families.
  • Coordinate and monitor services for youth and family across mental health, medical, educational, psychological, vocational, and social services.
  • Establish and maintain strong relationships with assigned foster parents and families of origin.
  • Assist in family reunification and stabilization efforts with the youth’s primary family or other permanent living arrangements.
  • Provide safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served.
  • Community-based program but may go into schools as well.

Family Service Coordinators with National Youth Advocate Program work in the community as part of an integral service team. Under the general supervision of the Clinical Supervisor or designee, this position works closely with youth, family, foster parents, and community partners, providing advocacy and support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
  • Must be available to work two evenings each week until 7:00PM when needed.
