Job summary

National Youth Advocate Program in Lafayette, IN is seeking an experienced professional to support children and families through advocacy, services coordination, and community partnerships. Under supervision, you will provide in-home services, transportation, counseling, and referrals to help youth remain safely connected to their families and community.

The role requires a Bachelor's degree in Social Work or a related field, a valid driver's license and willingness to work non-traditional hours

Qualifications

  • Bachelor's Degree in Social Work or comparable Human Services field from an accredited institution.
  • Must have night and weekend availability as needed.
  • Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record.
  • Willingness to work flexible and non-traditional hours in the service of families of origin and persons served.
  • Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers.
  • Capacity to remain objective and professional in all areas of job function.
  • Demonstrates tolerance and respect for the ideas and actions of others.
  • Ability to work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.

Responsibilities

  • Actively supports, represents, and extends the mission, vision, and values of the organization.
  • Provides in-home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to youth and family.
  • Travels daily to provide community-based services to youth and families in compliance with requirements.
  • Completes all required documentation in a timely manner.
  • Coordinates and monitors services for the youth and family in the community, including mental health, medical, educational, psychological, vocational, and social services.
  • Maintains relationships with foster parents and families of origin.
  • Assists in family reunification and stabilization efforts.
  • Provides safety assessment, crisis response, and behavioral stabilization services as required.

Position Summary

Under the general supervision of the Clinical Supervisor or designee, this position works to make a meaningful difference in the lives of the children and families served. This position works closely with youth, family, foster parents, and community partners, providing advocacyand support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
  • Activelysupports,represents,andextendsthemission,vision,andvaluesoftheorganization.
  • Provides in home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to the youth and family, meeting or exceeding established outcomes thresholds on behalf of persons served.
  • Travels daily, to provide community based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
  • Completesallrequireddocumentationinatimelymanner.
  • Completesallrequiredtrainingsinatimelymanner.
  • Coordinates and monitors services for the youth and family in the community, including but not limited to, mental health, medical, educational, psychological, vocational, and social services.
  • Regularlyreviewsandmodifiesyouthbasedplanstoassureconsistentprogressandsuccessonwrittenplangoals.
  • Establishesandmaintainsstrongrelationshipwithassignedfosterparentsandfamiliesoforigin.
  • Assists in family reunification and/or stabilization efforts with the youth’s primary family or other permanent living arrangement consistent with the youth’s case plan.
  • Providessafetyassessment,crisisresponse,andbehavioralstabilizationservicesasrequiredtoassuresafetyandstabilityforyouthserved.
  • Consistentlyachievesestablishedproductivitythresholds.
  • ParticipatesintheContinuousQualityImprovementactivitiesonamonthly/quarterlybasis.
  • Activelyparticipatesintheweeklysupervisionprocess.
Minimum Qualifications
