Job summary

Family & Children’s Agency (FCA) in Norwalk, CT seeks a FFT Family Specialist to support foster youth and families through case management, tailored interventions, and collaborative treatment planning.

Working with the FFT Family Therapist, you will coordinate services, complete documentation, conduct assessments, refer to community resources, and participate in court appearances, with a focus on stability and permanency.

Qualifications

• Bachelor's degree in Social Work or related field.
• Minimum 2 years’ experience in case management or related services.
• Experience with family systems and in-home services.

Responsibilities

• Provide home-based case management as an active member of the FFT FC team, collaborating with the Family Therapist and DCF.
• Conduct comprehensive initial assessments, identify key participants, and create relational and risk analyses within the family system.
• Complete Care4 and electronic health record documentation, including crisis management plans and safety planning.
• Regularly assess client engagement and levels of risk, coordinating with stakeholders to address barriers and ensure effective intervention strategies.
• Keep all relevant stakeholders informed of client progress and attend family court appearances and Administrative Care Reviews.
• Identify and refer children and families to community-based resources to enhance their support network.
• Demonstrate cultural sensitivity and understanding of diverse backgrounds and needs of the families served.
