Job summary

Saint Francis Ministries is seeking a Family Support Worker for Reintegration to provide in-home services, parenting and life skills development, and support to children and families toward permanency. Responsibilities include supervising visits, coordinating with the Case Manager, attending court proceedings when required, and traveling across Kansas to deliver case management services.

Qualifications

• High School diploma or equivalent.
• Prefer 2 years’ experience working with children/youth and families.
• Must pass a drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and an Adult Registry Check.
• FBI Fingerprint check.
• Lifting requirement of 50 lbs.
• Must have a valid driver’s license, acceptable driving record, and reliable transportation.

Responsibilities

• Supervises visits.
• Provides direct in-home services including parenting skills, life skills, child development and behavior management.
• Supports case plan goals through teaching, role modeling, and demonstration during regular visits.
• Participates in case staffing, family meetings, assessments, and team decision making.
• Refers families to community services and assists in arranging appointments and transportation.
• Completes and maintains documentation in the child/youth’s case record accurately and within established time frames.
• Works collaboratively with the Case Manager and participates in team meetings.
• Travels throughout the state of Kansas to provide case management services.
• On-call rotation outside regular hours to address urgent issues.
