Job description

Sign on Bonus. Full time days. The Family Medicine Physician will be responsible forcoordination of proactive care for patients with chronic conditions as well asidentifying wellness and/or preventative service for clinical patients. Thisposition will work closely with the clinical staff and will report to the SiteMedical Director.
