Job description

AdventHealth Well 65+ is seeking an experienced, Board Certified Family or Internal Medicine physician to join our team and provide personalized care to patients aged 65 and older. This practice focuses exclusively on Medicare Advantage patients and will be our newest location in Orange City, Florida.

At work, you will experience:

• More Time with Patients: Up to 40 minutes per patient for comprehensive, personalized care
• Smaller Patient Panel: Max of 700 patients
• Lower Daily Patient Census: 14-16 patients per day
• Clinic Support Team: Includes RN Case Management, 1.5 Health Coaches (Medical Assistants) per provider, Social Worker, Pharmacy, Care Coordinators, Welcome Coordinator, HCC Coding Support, and Administrator
