Hero Health Practitioners, LLC in Carthage, Missouri is seeking a dedicated Registered Nurse Practitioner to join our team. You will provide primary and acute care with a focus on family practice and collaborate with our physician team to deliver high‑quality patient care.

You will conduct exams, develop treatment plans, prescribe medications, monitor responses, and educate residents and families about wellness and prevention in a long‑term care setting.

Qualifications

• Minimum 2+ years NP experience.
• Experience in Family Practice, Adult, Geriatrics, or Long-Term Care.
• Familiar with Long-Term Care EHR systems such as PointClickCare.
• Knowledge of CMS Skilled Nursing Facility regulations.
• Familiarity with value-based care models and geriatric prescribing principles.

Responsibilities

• Conduct medical exams, take medical histories, and diagnose and treat common illnesses and injuries.
• Order and interpret laboratory tests and imaging studies as needed.
• Develop and implement individualized treatment plans, taking into account patient needs, preferences, and health status.
• Prescribe medications and therapies, and monitor patient response to treatment.
• Educate patients and families about healthy habits, disease prevention, and wellness strategies.
• Stay current with changes in medical policies, procedures, and regulations.
